Care Initiatives provides compassionate, individualized care and support for older adults and their families throughout Iowa. Services include skilled nursing care, assisted and independent living, hospice care, and physical, occupational, and speech rehabilitation. Care Initiatives supports individuals through changing health needs with a focus on quality of life, dignity, and personalized care.

Comfort Keepers provides in-home care and support to help seniors and adults with disabilities remain safely and comfortably in their own homes. Services may include companionship, help with daily activities, transportation, respite care, specialized care, and Alzheimer’s and dementia support. Their caregivers focus on promoting independence, quality of life, and meaningful connection while providing support tailored to each individual’s needs.

Our Mission

To provide comprehensive coordinated care to individuals with life limiting illnesses and to their families and to support them in the process of death and bereavement.

Our goal is to provide palliative care that addresses the physical, psychological, and spiritual dimensions of service for the terminally ill as an integrated whole.

Essence of Life Hospice in Iowa focuses on caring, not curing, and is provided in the patient’s home or other facility. Members of the our team make regular visits to assess the patient and provide additional care or other services. We pride ourselves on providing the highest quality care. We are on-call 24 hours a day, seven days a week.

Our team develops a care plan that meets each patient’s individual needs for pain management and symptom control. The team generally consists of:

Outpatient Care

Our outpatient services include:

• Care in a patient's home, nursing home, hospital, or at the Iowa River Hospice Home 
• Our team will travel within a 50-mile radius
• Help is available 24 hours a day, seven days a week
• Hospice care to anyone who qualifies, regardless of age, illness, or ability to pay
• Grief support to family members and loved ones during and after hospice care

Admission, Referral, & Payment

Referral Process

Anyone can make a hospice referral. Call 641-753-7704. Our Referral/Admissions Nurse will contact the patient's primary physician for the required admission order. A referral visit with the patient/family will be scheduled in a timely manner. 

We offer hospice care and comfort tailored to you and your family’s needs. Our goal is to help you focus on your quality of life, and to not be limited by your diagnosis. At St. Croix Hospice, we’ll ensure that hospice care is provided to you in a way which brings dignity, respect, and compassion on your terms.
